Class and mechanism: Warfarin inhibits the synthesis of biologically active forms of the vitamin K-dependent clotting factors: II, VII, IX and X, as well as the regulatory factors protein C, protein S and protein Z. Other proteins not involved in blood clotting, such as osteocalcin, or matrix Gla protein, may also be affected.

What side effects may occur?

The two most serious side effects are bleeding and necrosis (gangrene) of the skin. Bleeding can occur in any organ or tissue. Bleeding around the brain can cause severe headache and paralysis. Bleeding in the joints can cause joint pain and swelling. Bleeding in the stomach or intestines can cause weakness, fainting spells, black tarry stools, vomiting of blood, or coffee ground material. Bleeding in the kidneys can cause back pain and blood in urine.
Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
    Other less serious side effects are more likely to occur, such as:
  • excessive gas or bloating;
  • diarrhea, nausea, or vomiting;
  • hair loss; or
  • loss of appetite, weight loss.
Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome.

Drug Interactions

Coumadin interacts with many other drugs, and these interactions can be dangerous, even fatal. Tell your doctor about all the prescription and over-the-counter medications you use. Some medications can enhance the action of Coumadin and cause excessive blood thinning and life-threatening bleeding. A few examples of such medications include Aspirin, Tylenol, Oxandrin, certain vitamins, and antibiotics.
    Coumadin can interact with the following herbal (botanical) products:
  • bromelains;
  • coenzyme Q10;
  • cranberry;
  • danshen;
  • dong quai;
  • garlic;
  • ginkgo biloba;
  • ginseng; or
  • St. John's wort.
Do not use any of these products without first asking your doctor. Some of these herbal products can cause you to bleed while you are also taking Coumadin.

Additional Information

Tell your doctor and pharmacist if you are allergic to warfarin, aspirin, tartrazine (a yellow dye in some medications and processed foods), or any other drugs. Tell your doctor and pharmacist what prescription and nonprescription medications you are taking, especially other heart medications; antibiotics; aspirin and other non-steroidal antiiflammatory drugs such as ibuprofen (Advil, Motrin) and naproxen (Aleve, Naprosyn); cimetidine (Tagamet); medications for cancer, depression, diabetes, digestive problems, epilepsy, gout, high cholesterol, and thyroid problems; and vitamins. Many medications interfere with the effectiveness of warfarin. It is important that you tell your doctor every medication that you take, including nonprescription medications. Do not take any new medications without talking to your doctor.
